Please print and bring these completed forms to your appointment or arrive 15 minutes early to fill them out in the office.
Return patients who have not been seen in 3+ years are required by Oregon law to be re-admitted as a new patient.
Insurance Policy
Please note: we cannot see patients who are using any type of Workers' Compensation or Automobile/Motor Vehicle insurance.
The physicians at Northwest Foot and Ankle LLC do not participate in any type of insurance program or agreement. We will not bill insurance. We are considered an "out of network" provider for every insurance plan.
Many insurance plans will not pay for visits to "out of network" providers. Also, many plans will not reimburse you for the costs of seeing a provider outside their network.
If you have Medicare, Oregon Health Plan (OHP), CareOregon, HealthShare Oregon, FamilyShare, Tricare, Medicaid, and/or any other type of govt. issued health plan, your visit will not be covered and they will not reimburse you.
You may or may not receive a Superbill for your visit. Receipt of a Superbill is not an indicator that your insurance will reimburse you. Our Superbill is meant to be a receipt of your visit and/or a record for your files.
Please contact your insurance company directly if you have questions on your coverage. We do not communicate with insurance companies and do not have information on your benefits.
Medical Appointment Pricing
- New patient appointment: $200-552
- Return patient appointment: $78-324
- Appointment deposit (new patient): $200
Specialty medical procedures and purchases of any over-the-counter products are an additional cost.
Due to Oregon law, patients who have not been seen by a NWFA physician in the past three (3) years must be re-admitted as a new patient.
If you do not answer or return our call to collect a deposit within 48 hours, we will cancel your appointment.

- Return patient remote consultation: $180
- Appointment deposit (new patient remote): $240
Patients cannot submit remote consultations to insurance.
